New Patients
We are currently accepting new patients with referrals from primary care physicians. To schedule a new patient appointment, please call our office at (413) 734-5661.
Please plan to arrive 15 minutes early to complete new patient paperwork. You may also call ahead to request forms be mailed or faxed to you.
What to bring to your first appointment:
Photo ID
Insurance card(s)
List of current medications and dosages
Referral from your primary care physician
Any relevant lab results, X-rays, or records from prior providers
Patient Portal
Access your medical records, test results, and appointment summaries through our online patient portal at www.myhealthrecord.com/portal.
Interpreter Services
Language assistance is available at no cost to patients who need it. Please inform our staff when scheduling if you require interpreter services. We serve patients in Spanish, Portuguese, and other languages spoken in our community.

Monday through Friday: 7:30 AM - 4:00 PM
We are closed on weekends and major holidays.
